Legal Name Change

Legal Name ChangeBefore a legal name change can be started you must go to a police station or RCMP to get fingerprints done.

You can come into our office to pick up the application form, the form cannot be printed online. All pages to the application form must be present when coming to the office.

Documents:

  • When the birth, marriage, or death occurred in Alberta the certificates are not required as the information will be verified directly from the Vital Statistics records.
  • When the birth, marriage, or death occurred outside of Alberta, copies of the official government birth, marriage, and death certificates must be submitted with this application.
  • When the birth, marriage, or death occurred outside of Canada, copies of marriage and death certificates, a copy of a permanent resident card, Canadian Citizenship card (both sides) or immigration document must be submitted with this application.
